The CCOE at NorthPoint
is a federally qualified health center serving north Minneapolis and
surrounding area. Selected as a CCOE in 2001, NorthPoint is taking a
holistic approach to women’s health by coordinating all aspects of a women’s
health throughout the life span. This program addresses social, economic and
cultural
influences that often act as barriers to quality health care for women.
NorthPoint seeks to increase women’s health knowledge and empower women to
take care of their health by identifying barriers and responding to issues
of concern to underserved women. Our providers improve the physical, mental
and social well-being of our clients. Since women often take on the role of
managing the health of the family, this leads to healthier local
communities.
At the CCOE at NorthPoint Health & Wellness Center, we work with you to make health happen in North Minneapolis. You and your family can rely on our capable health and human services professionals to provide thorough and comprehensive care. We integrate quality community health care with human services to improve lives.


The National Community Centers of Excellence (CCOE) in Women’s Health program was developed by the Office on Women’s Health within the Department of Health and Human Services (DHHS) in 2000.
Through the CCOE program, community organizations, like NorthPoint Health & Wellness Center, partner to improve health outcomes for women and girls using a holistic approach. The program strives to integrate health services and develop models of health care which will advance women’s health. The National Centers of Excellence in Women’s Health serve as national models to provide excellent health care for women.
Currently, there are 14 CCOE’s in federally qualified health centers and community hospitals throughout the country. Hennepin County Department of Primary Care—NorthPoint Health & Wellness Center is proud to be among them.
