· Diabetes Collaborative- diabetes education classes

· North High School Clinic, a school-based clinic offering primary health -care for students at Minneapolis North High School

The CCOE at NorthPoint can connect you with a full range of health and human services including: Medical Services, Mental Health Services, Dental Services and Human Services. You can rely on us for comprehensive, high-quality care. Our clinic is fully equipped to provide a wide range of medical services.

 The CCOE offers health programs and tools designed for women to help them make positive choices regarding their health including:

· Our Healthy Heart Program offers a free initial heart risk
assessment and a nurse coordinator who will help you make
heart healthy choices and stay on track with your personalized healthy heart program.

· OB Care Coordination Program, a program to provide care coordination for  pregnant women to ensure a healthy birth outcome

· Quarterly women’s health newsletters

· Assistance with insurance plans                                            

· Use of the Women’s Resource Center to browse our library and have a cup of tea or coffee in a women-centered, safe environment
